As a Neighborhood Advisor I will:

  • Assist Sage community in obtaining the services they need to remain independent in their homes

  • Promote awareness of community services

  • Make referrals and connect Sage community with services

  • Follow-up to ensure that the services have been received

 

The services of a Neighborhood Advisor are free and confidential. An appointment is needed to meet with me via phone or Zoom call.  Upon reopening  we will be able to meet in person.  I will assist with the completion of medical applications and other forms as well as assist in locating services related to:

  • Health Care

  • Legal Advice

  • Transportation

  • Housing

  • Nutrition and Exercise

  • Socialization

  • Caregiver Information

I can also provide information about income-stretching programs such as:

 

  • Social Security Income

  • SNAP (Food Stamps)

  • Employment

  • EPIC

  • Home Energy Assistance Program (HEAP)

  • IT-214 Form/· Property Tax Reduction
