As a Neighborhood Advisor I will:

Assist Sage community in obtaining the services they need to remain independent in their homes

Promote awareness of community services

Make referrals and connect Sage community with services
-
Follow-up to ensure that the services have been received
The services of a Neighborhood Advisor are free and confidential. An appointment is needed to meet with me via phone or Zoom call. Upon reopening we will be able to meet in person. I will assist with the completion of medical applications and other forms as well as assist in locating services related to:

Nutrition and Exercise

Socialization

Caregiver Information
I can also provide information about income-stretching programs such as:
